State Certification Extensions and Reinstatement of Expired Providers

Here is an important update on the reinstatement of previously certified providers as outlined by the NYS DOH BEMST office on an April 9th conference call. These efforts are in response to the ongoing COVID-19 pandemic.
1) Any provider who expired between 9/1/2019 and now have already received a 1 year extension on their certification. There is no need to do anything and agencies can confirm certifications via the Health Commerce System. Those who were expired will have an expiration date of 12/31/2020. They will be eligible for renewal (submit at least 45 days prior to 12/31/2020.)
2) Providers who expired between 1/1/2019 and 8/31/2019 may apply for provisional certification at their last level of care. If there are no issues, the provider will be issued a verification letter. Their expiration date will be 12/31/2020 and they will be eligible for renewal (submit at least 45 days prior to 12/31/2020.)
3) Providers who expired between 1/1/2015 and 12/31/2018 may apply for provisional certification at the CFR or EMT level only, regardless of last certification level. If there are no issues, the provider will be issued a verification letter. Their expiration date will be 12/31/2020 and they will be eligible for renewal (submit at least 45 days prior to 12/31/2020.)

Expired individuals (see 2 and 3 above) must apply for reinstatement at https://apps.health.ny.gov/pubpal/builder/survey/expired-verification.
Please note that any advanced provider who was expired, received a reinstatement, and wishes to become re-credentialed by Mountain Lakes, must complete an application and receive an endorsement from their agency and agency medical director. See links below.
